PROJECT FUTURE
Project Future is a health education and support program that provides services to pregnant or parenting teens and their children. Weekly parenting groups are offered in school or community settings. Case management services include home assessments (i.e., home safety and detection of developmental delay), hospital visits, and information and referrals to community resources. The program offers services to teens in the Metro Atlanta area but specifically, those who reside in the City of Atlanta or attend Atlanta Public Schools. The program provides parent education groups and one-on-one sessions with teens that are pregnant or parenting children ages
0-5.
PROJECT L.E.A.P (Learning Effective and Applicable Parenting)
Project LEAP empowers its participants to “leap” towards positive parenting. The program provides home-based parent training sessions implemented by a trained Family Intervention Specialist (FIS) and a bi-weekly parent education and support group. The FIS supports parents in identifying and obtaining services needed to attain and maintain a healthy and safe environment for themselves and their families. The education and support group offers parents an opportunity to support each other, network, obtain information and discuss the challenges of parenting in a non-judgmental environment.
TAKE CHARGE HEALTH AND WELLNESS PROGRAM
The Take Charge program is a health and wellness program designed to increase health knowledge and healthy living among individuals with disabilities.
The Take Charge program offers its participants:
- Weekly one on one health sessions
- A trained health educator
- Instructions on preparing healthy meals and snacks
- Tips for stress management
- Strategies to increase physical activity
- Steps to positive decision making
Participants are enrolled in the Take Charge program for 8-10 weeks depending on the needs of the individual. The weekly sessions cover topics such as nutrition and diet, exercise, stress management, decision-making and communication. The goal of the program is to engage participants in leading a healthy lifestyle to increase positive health outcomes.
